A valid provider order may be required for insurance billing. Patients are responsible for any co-pays, deductibles, or non covered services as determined by their insurance carrier.
Self-pay pricing is available for services not covered by insurance, including DOT services, advance services, employer screenings, and anyone that does not have an insurance plan.

DOT (Department of Transportation) testing is federally regulated drug and alcohol testing required for safety-sensitive employees, such as commercial drivers. These tests follow strict guidelines set by the Department of Transportation to ensure compliance, accuracy, and confidentiality.
Non-DOT testing applies to employers, schools, courts, and individuals who require drug or alcohol testing but are not regulated by federal DOT rules. These tests are more flexible and may be customized based on workplace policies or individual needs.
We provide certified, confidential, and compliant DOT and Non-DOT testing, including pre-employment, random, post-accident, reasonable suspicion, and return-to-duty testing.

If you prefer not to establish care with our provider, our mobile lab services may be a better option. Mobile services allow us to collect specimens at your home, workplace, or other convenient location without requiring an in-office provider relationship.

Appointments canceled or rescheduled with less than 24 hours’ notice may incur a cancellation fee. This fee helps cover scheduling, staffing, and travel costs reserved for your appointment.
